This expanding nut secures the door grab handle to the interior of the door. It lives in the door sheet metal behind the door panel and the grab handle is screwed into it.

These nuts don't usually wear out but sometimes if you get a gorilla with a screwdriver installing your interior, you may need to replace these nuts.

There are two per handle.
